You may close your account by submitting a Request for Final form.

A

Orders to close an account will be processed on the first business day (excluding holidays) after it is received or later, but not more than three business days after the order is received.

A

The requested date for cut-off must be the same date you place your order or later. We cannot process orders with a retroactive cut-off date. We can process orders with a future cut-off date.

A

Only the account owner is authorized to close the account.

A

In the event the account owner is deceased, a certified copy of the death certificate is required for another party to close the account.

A

If the account owner cannot close the account due to unforeseen circumstances, a durable power of attorney is required for another party to close the account.

A

No orders to close an account will be accepted over the phone.

A

When submitting your order to close your account by any method, other than in person at the SWSA office, it is the account owner’s responsibility to follow up on a submitted order to verify that the order has been received.

A meter reading will be taken and you will receive a final bill statement for water registered through the meter since your last billing. The final bill statement will be mailed to the forwarding address you provide on your order.

Final bill balances are NOT automatically deducted from your checking account if you participate in Auto Pay. A final statement will be mailed to the forwarding address you provide.

A

Any final bill balance 30 days or more past due may be sent to a collection agency for collection action.

Request for Final

If this form is submitted by e-mail, you will receive a reply acknowledging receipt of request. If you do not receive a confirmation e-mail by the end of the first business day after you submitted your request, please call the SWSA office at 334-298-6342. You may fax this form to 334-298-6412 for processing or mail to SWSA, P.O. Box 727, Smiths Station, AL 36877. You are responsible for following up to verify that the office received your request. All Requests for Final are processed on the next business day after it is received or later, but not more than three business days after it was received. Business hours are Monday-Friday(excluding holidays) 8:30 a.m.-5:00 p.m. Eastern time. Incomplete information will void the processing of your request.

  • By signing below, I certify that I am the account owner of the account referenced above. I request that my water service be discontinued as of the cut-off date indicated above. I understand that a final meter reading will be taken and I will receive a bill for water registered through the meter since my last billing. I understand that I am responsible for this bill and agree to pay the balance in full upon receipt. I further understand if I was previously enrolled in Auto Pay (automatic bank draft) that the final bill will not be automatically deducted from my checking account and that any unpaid balance may be sent to a collection agency for collection action.
